If you are seeking a primary care physician you can visit our rural health clinic. Our rural health clinic practitioners provide chronic care management, general wellness maintenance, acute illness treatment, and other medical services you might need from a primary care physician.

When a patient receives active but short-term treatment for a severe injury or episode of illness, an urgent medical condition, or during recovery from a surgery, it is known as acute care. It is considered the direct opposite of long-term care or chronic care.

Services defined as Acute Care are normally delivered by coordinated teams of healthcare professionals that specialize in a broad range of both medical and surgical specialties. The typical goal of acute care revolves around discharging patients as soon as they are determined to be healthy and stable.

Often, one may hear the terms outpatient or inpatient used when referring to medicine or healthcare. “Inpatient” means that the procedure requires the patient to be admitted to the hospital, so that the patient can be closely monitored during the procedure and recovery. “Outpatient” means that the procedure does not require hospital admission and may also be performed outside the premises of a hospital.

After surgery or hospitalization, some patients require additional care. Our Swing Bed Program is 24-hour skilled nursing care plus the benefit of rehabilitation therapies to help patients transition home or to a long-term care facility. Swing Bed can be utilized to help rehabilitate from surgery, illness, or an accident. It acts as a bridge between the patient’s stay at the hospital and their return home to give them time to heal.
